What Should I Bring To My Appointment?

Please have the following items ready when you check in at the front desk. This way, your appointment can proceed as smoothly as possible.

  • Information about the patient, including full name, date of birth, home address, phone number, and social security number.
  • A valid form of identification
  • Current insurance card
  • A list of medications you currently take and allergies you may have
  • Contact information for your primary care physician
  • Completed New Patient Forms

For your convenience, we have many of our New Patient Forms available below. Please feel free to print and fill out your forms before arriving at your first appointment in order to shorten your wait time.

Patients can print and fill out these forms ahead of time and bring them to our office on the date of their appointments. If patients wish to submit these forms electronically, please send the filled out / scanned forms to [email protected].

What Can I Expect At My Appointment?

Swift Health Urgent Care is dedicated to providing outstanding medical services to each patient that comes through our doors.

Once you arrive, one of our caring staff members will check you in at the front desk, and from there, we will do our best to ensure that your time in the waiting room is short.

We will address your needs and provide you with outstanding medical care in a timely manner, allowing you to complete your appointment and start feeling better as soon as possible.

Insurance and Payment Types

Though we accept most insurance plans and payment methods (Visa, MasterCard, Discover, etc.), we recommend that patients contact our team ahead of time to ensure that everything runs smoothly. If you have any questions, please contact us.

When Should I Arrive?

Please try to make it to our office at least 15 minutes before your scheduled appointment. This way, if there is any necessary paperwork that has not been completed, you will have time to fill it out before being seen. Our goal is to help you complete your visit as quickly and conveniently as possible.

Our primary doctor's office closed early today so we decided to go to this urgent care. The staff was very professional and personal with us. My son asked if we could change our provider to this location (which means he was comfortable here) but I was informed by the staff member that they have a provider in Morrow. We will be using their doctor soon as our primary provider. The doctor was great with explaining the issue and electronically sending the prescription to our pharmacy of choice. I will be back if I ever need an urgent care facility.
